Care Coordinator

Job Summary

A company is looking for a Care Coordinator.

Key Responsibilities

• Serve as the primary point of contact for concierge care and closed-loop referral follow-up
• Facilitate and manage referrals for Intensive Treatment Services (IOP and PHP), ensuring timely outreach and placement
• Develop and maintain relationships with external referral sources and coordinate with internal teams to understand client needs

Required Qualifications

• High school diploma or equivalent required; Associate's degree in a healthcare-related field preferred
• Minimum 2-3 years of experience in care coordination or referral management within behavioral healthcare or community health settings
• Familiarity with higher levels of care (IOP, PHP, inpatient) and behavioral health systems
• Experience with electronic health records and case tracking systems
• Experience working remotely and/or with virtual care models is a plus
