Job Summary

A company is looking for a Bilingual Continuous Care Coach - COA (Remote).

Key Responsibilities

• Assess, coordinate, and manage care needs of patients with chronic eye diseases
• Develop and implement personalized care plans in collaboration with patients, families, and healthcare providers
• Facilitate access to care and community resources while providing education to support self-management

Required Qualifications

• Bilingual in English and Spanish
• 2+ years of work experience in eye care
• Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) required
• Proficient with Electronic Medical Record (EMR) systems, preferred
• Ability to work independently in a remote environment
