Job Summary

A company is looking for a Care Coordinator to support clinicians and enhance patient care in a remote setting.

Key Responsibilities

• Manage EMR onboarding and intake processes, ensuring accurate completion of patient forms
• Coordinate care navigation, including outreach to external providers and managing patient referrals
• Support daily operations and group enrollment management for skills-based treatment programs

Required Qualifications

• Master's-level behavioral health clinician, including pre-licensed professionals
• Deep understanding of trauma-informed care and best practices
• Proven experience in case management or care coordination
• Ability to adapt to rapid changes and develop new workflows in a startup environment
• Proficiency with electronic health records (EHRs) and healthcare software
