A company is looking for a Care Coordinator, Complex Care.
Key Responsibilities
- Conduct health risk assessments through calls to members to understand their health and needs fully
- Coordinate with provider offices and home health agencies to ensure members receive necessary support
- Check in with members over the phone to assess their well-being and confirm they have received the assistance they need
Required Qualifications
- Minimum education: High school diploma or GED equivalent
- 2+ years of experience in healthcare administration or home care
- Proficient in technology (experience with Gmail/G Suite)
- Comfortable working in an interdisciplinary team environment
- Fully bilingual (English/Spanish) is preferred
