Care Coordinator

Job Summary

A company is looking for a Care Coordinator, Complex Care.

Key Responsibilities

• Conduct health risk assessments through calls to members to understand their health and needs fully
• Coordinate with provider offices and home health agencies to ensure members receive necessary support
• Check in with members over the phone to assess their well-being and confirm they have received the assistance they need

Required Qualifications

• Minimum education: High school diploma or GED equivalent
• 2+ years of experience in healthcare administration or home care
• Proficient in technology (experience with Gmail/G Suite)
• Comfortable working in an interdisciplinary team environment
• Fully bilingual (English/Spanish) is preferred
