Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Perform outreach to members via phone to assist with care plan next steps and resources
  • Connect members to health plan and community resources for quality customer care
  • Document and maintain non-clinical member records in compliance with regulations
Required Qualifications
  • High School diploma or GED required
  • 1 - 2 years of related experience preferred
  • Knowledge of health plan activities and resources
  • Understanding of Social Determinants of Health (SDOH)
  • Ability to comply with policies and standards
