Job Summary

A company is looking for a Spanish Speaking LPN Nurse (REMOTE) in Colorado.

Key Responsibilities

• Conduct comprehensive assessments of patients' health needs
• Collaborate with healthcare teams to develop and update care plans
• Educate patients on chronic disease management and coordinate care across providers

Qualifications & Requirements

• Active Colorado Licensed Practical Nurse (LPN) license in good standing
• Fluency in Spanish and English (oral and written)
• At least 2 years of experience in patient care or chronic disease management
• Experience with Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) programs preferred
• Proficiency in EHR systems and familiarity with RPM tools
