Job Summary

A company is looking for a Registered Nurse Patient Navigator.

Key Responsibilities

• Serve as a liaison between patients, families, and healthcare providers to ensure effective communication
• Monitor and manage patient symptoms and complex care to minimize hospital readmissions
• Provide ongoing education, resources, and referrals to patients and caregivers

Required Qualifications, Training, and Education

• Registered Nurse multistate or Florida single state licensure required
• Three (3) years or more of experience as a Registered Nurse required
• Two (2) years or more of oncology experience required
• Bachelor of Nursing preferred
• Certification as an Oncology Certified Nurse (OCN) preferred
