Delaware Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for pediatric patients.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Develop ongoing care plans and collaborate with providers to address unmet needs of members
• Provide education and resource support to members and their families regarding healthcare options and services

Required Qualifications

• Active RN (Registered Nurse) license
• Pediatric or neonatal experience
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• State Licensure and/or Compact State Licensure required
