Job Summary

A company is looking for a Health Coach (Cardiology) to join their team remotely.

Key Responsibilities

• Provide patient education on chronic disease management, particularly for heart failure
• Establish and monitor short-term and long-term health goals with patients
• Collaborate with the clinical team to ensure optimal patient outcomes and maintain patient relationships

Required Qualifications

• Health coach certification from an accredited program; bachelor's degree in health sciences preferred
• Experience in a medical office or healthcare environment; cardiology coaching experience is a plus
• Familiarity with telehealth practices is advantageous
• Proficiency in electronic health records and Microsoft Office applications is preferred
• Bi-lingual fluency in English and Spanish is highly preferred
