Health Advocate

Job Summary

A company is looking for a Health Advocate to support patients with chronic kidney disease through telephonic outreach and care coordination.

Key Responsibilities

• Manage multiple technology systems and document interactions accurately
• Build rapport with members through daily outbound and inbound calls while providing excellent customer service
• Assist members with healthcare appointment scheduling and inform them about the Kidney Services program

Required Qualifications

• High School Diploma / GED or equivalent work experience
• Must be 18 years of age or older
• 2+ years of prior medical or healthcare experience
• Proficiency in Microsoft Word, Excel, and Outlook
• Able to work 40 hours per week, including some evening hours
