Key Responsibilities
- Maintain knowledge and use critical thinking to triage and delegate care effectively
- Contact members regarding out-of-range vital signs and assess their immediate health needs
- Collaborate with clinical services and coordinate member education and resources
Required Qualifications
- Proficiency with technology and web-based applications
- Ability to work closely with members and care teams
- Experience working with diverse populations
- Knowledge of chronic disease management
- Understanding of Medicaid and/or Medicare regulations
