Job Summary

A company is looking for a Health Coach - Remote.

Key Responsibilities

• Engage and enroll eligible participants in health programs through telephonic calls
• Provide individualized coaching to promote behavior change and improve health outcomes
• Coordinate care with a multi-disciplinary team and track member progress

Required Qualifications

• Bachelor's degree in a health-related field required
• 1-2 years of related experience preferred, especially in customer service
• Experience with benchmarks and contact center metrics preferred
• Proficiency in computer data entry and Microsoft Office
• Ability to work independently and adapt to changing schedules
