Key Responsibilities
- Make outbound and receive inbound calls to assess members' health status
- Identify gaps in treatment plans and provide patient education for self-management
- Coordinate care and connect members to resources for comprehensive support
Required Qualifications
- Current, unrestricted RN license in the state of employment
- 3+ years of experience in a hospital, acute care, or direct care setting as a registered nurse
- Proven ability to type and navigate a Windows-based environment
