Job Summary

A company is looking for a Population Health Pharmacy Coordinator.

Key Responsibilities

• Manage and analyze information from medical records and technology systems to support quality measures
• Provide direct patient outreach to assist in medication management and monitor disease adherence
• Collaborate with healthcare providers to ensure comprehensive care and compliance with regulatory requirements

Required Qualifications

• High School Diploma/GED
• 2 years of Pharmacy technician or pharmacy intern experience or 2 years as a Certified Pharmacy Technician (CPhT)
• Familiarity with Microsoft Excel and basic documentation skills
• Knowledge of brand name/generic medications and medical terminology
• CPhT certification upon hire
