Key Responsibilities
- Assess member needs and oversee the development of client care plans
- Advocate for clients with healthcare professionals and connect them to necessary social services
- Maintain documentation and evaluate client progress while ensuring program compliance
Required Qualifications
- 2+ years of experience as a care manager, care navigator, or community health worker
- Experience working in a remote setting
- Working knowledge of government and community resources related to social determinants of health
- General computer skills and familiarity with Google Workspace and MS Office
- Bilingual (English/Spanish) is a plus
