Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Behavioral Health Case Manager - Remote.

Key Responsibilities

• Assist consumers using clinical skills and provide appropriate referrals for psychiatric and community resources
• Manage discharge planning and transitions between care settings to ensure quality outcomes
• Collaborate with providers and participate in treatment plan discussions and performance review meetings

Required Qualifications

• Licensed Master's degree in Psychology, Social Work, Counseling, or a related field; or RN with 2+ years of behavioral health experience
• Active, unrestricted clinical licensure in the state of residence
• 2+ years of experience in a related mental health setting
• Proficient in MS Office Suite
• Ability to work from a secure, dedicated workspace at home
