Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Behavioral Health Case Manager.

Key Responsibilities

• Screen and assess patients for common mental health disorders and facilitate patient engagement
• Provide brief behavioral interventions and track treatment response and patient progress
• Collaborate with a multidisciplinary team to develop personalized treatment plans and coordinate care

Required Qualifications

• Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or equivalent
• Minimum 1-year of direct clinical experience working with children and adolescents
• Experience in delivering brief psychosocial evidence-based treatments
• Experience with care coordination in a team-based healthcare setting
• Working knowledge of differential diagnosis of common mental health disorders
