Job Summary

A company is looking for a Care Manager, LPN.

Key Responsibilities

• Deliver telephonic care management for patients in Value-Based Care Programs
• Develop patient-centered care plans and educate patients and caregivers
• Communicate patient health updates and coordinate care with providers and support teams

Required Qualifications, Training, and Education

• Active, unrestricted compact LPN license
• 2+ years of nursing experience in various care settings
• Knowledge of pathophysiology and treatment protocols for common health diagnoses
• Proficient computer skills and ability to work in a remote environment
• Experience with clinical data to guide decision-making
