Key Responsibilities
- Deliver telephonic care management for patients in Value-Based Care Programs
- Develop patient-centered care plans and educate patients and caregivers
- Communicate patient health updates and coordinate care with providers and support teams
Required Qualifications, Training, and Education
- Active, unrestricted compact LPN license
- 2+ years of nursing experience in various care settings
- Knowledge of pathophysiology and treatment protocols for common health diagnoses
- Proficient computer skills and ability to work in a remote environment
- Experience with clinical data to guide decision-making
