Job Summary

A company is looking for a Social Worker Care Manager (Hybrid) to service patients in the Frederick, MD area.

Key Responsibilities

• Support mental and physical health care for an assigned patient caseload
• Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care
• Document patient progress and treatment recommendations in EHR and other required systems

Required Qualifications

• Minimum of MSW
• Preferred Licensure or provisionally licensed
• Behavioral health experience and knowledge
• Must be a licensed driver with an insured automobile in good working order
• Comfortable utilizing Excel and Word for assessments and productivity tracking
