Job Summary

A company is looking for a Clinical Pharmacist, Advanced Therapeutics- Cardiometabolic and MWL (Ambulatory).

Key Responsibilities:

• Provide comprehensive medication management services including medication reviews and adherence monitoring
• Design and implement targeted medication and disease management programs to optimize patient outcomes
• Monitor drug therapy for appropriateness and document clinical interventions in electronic health records

Required Qualifications:

• Doctor of Pharmacy (Pharm.D) degree or Bachelor of Science in Pharmacy (BSPharm) with 3 years of hospital experience
• Licensure with the Massachusetts Board of Registration in Pharmacy
• Preferred: ASHP Accredited Residency in Pharmacy Practice (PGY1)
• Preferred: Pharm.D. degree with a minimum of 1 year of hospital experience
