Michigan Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Develop and coordinate ongoing care plans in collaboration with healthcare providers and community resources
• Monitor member progress and provide education to members and their families regarding healthcare options and benefits

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in nursing
• Licensed as a Registered Nurse in Michigan or a Compact State
• Experience in physical health and obstetrical nursing is preferred
