Multi-State Licensed Care Management Nurse

Job Summary

A company is looking for a Care Management Registered Nurse.

Key Responsibilities

• Continuously monitor patient vitals, symptoms, and lab results to identify care gaps
• Create and manage personalized care plans in alignment with treatment goals
• Conduct virtual follow-up appointments and document clinical interactions thoroughly

Required Qualifications

• Multi-state RN Compact State Licensure and RN NY Licensure
• Associate Degree in Nursing Science
• 5+ years of clinical experience in a chronic care management program
• Experience with EMR systems, preferably Athena and EPIC
• Competency in patient assessment and nursing processes
