Job Summary

A company is looking for a Care Manager, Complex & Disease Management.

Key Responsibilities

• Provide care management and coordination for high-risk members, including assessments and identifying gaps in care
• Engage with members and their treatment teams to establish goals and improve health care outcomes
• Ensure compliance with care management processes and maintain accurate documentation in the Electronic Medical Records System (EMR)

Required Qualifications, Training, and Education

• Bachelor's degree
• Current active RN license required
• 4 - 6 years of clinical experience
• CCM certification preferred
• Experience in case management, care coordination, or managed care
