Oklahoma and Illinois Licensed Clinical Pharmacist

Job Summary

A company is looking for a Clinical Pharmacist to provide patient care and support clinical teams in medication management and chronic disease support.

Key Responsibilities

• Provide one-on-one counseling to patients and caregivers on medication use and adherence
• Collaborate with healthcare teams to develop patient-centered care plans and educate on safe prescribing practices
• Utilize clinical data to identify high-risk patients and support quality improvement initiatives related to medication management

Required Qualifications

• Doctor of Pharmacy (PharmD) degree required
• Active pharmacist license in the State of OK and IL preferred, or ability to obtain a license in these states
• Minimum 3 years of clinical pharmacy experience, preferably in ambulatory care or population health
• Direct patient care experience required; background in FQHCs or working with underserved populations strongly preferred
• Familiarity with value-based care and chronic disease management
