Oregon Licensed Behavioral Health Coach

Job Summary

A company is looking for a Behavioral Health Coach to develop and implement coaching programs and healthcare navigation support for members.

Key Responsibilities

• Provide one-on-one health coaching to members managing chronic health conditions through various communication channels
• Educate members using evidence-based guidelines to help them set and achieve health-related SMART goals
• Collaborate with healthcare teams to improve healthcare experiences and outcomes while reducing costs

Required Qualifications, Training, and Education

• Current Oregon license as a Clinical Social Worker (LCSW), Professional Counselor (LPC), or other mental/behavioral health accreditation
• Bachelor's degree, preferably in a healthcare-related field
• Strong cultural competence and understanding of Diversity, Equity, and Inclusion (DEI) concepts
• Thorough knowledge of health behavior change theory
• Experience with Microsoft Office products and ability to learn proprietary databases
