Oregon Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to achieve optimal healthcare outcomes
• Develop and coordinate ongoing care plans, collaborating with providers and community resources
• Monitor member progress and adjust care plans as necessary to meet individual goals

Required Qualifications

• Active RN license in the state of Oregon
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Knowledge of state, federal, and third-party payer regulations
• Experience in providing education on healthcare processes and options to members and families
