Pennsylvania Licensed Care Management Manager

Job Summary

A company is looking for a Manager Integrated Care Management.

Key Responsibilities

• Plan, organize, and direct the development and implementation of Case Management in accordance with the DSNP Model of Care
• Ensure compliance with NCQA, State, and CMS standards while monitoring departmental goals and objectives
• Direct case management activities to comply with the Medicare Model and support the corporate Medical Management Strategy

Required Qualifications

• Bachelor's Degree required
• Active PA (or compact) RN licensure required
• 8 to 10 years of progressive experience in healthcare delivery, with at least 5 years in managed healthcare
• Minimum of 5 years managerial experience, with prior case management management preferred
• Minimum of 1 year project management experience, preferably at the management level
