Job Summary

A company is looking for a Population Health Strategy Program Lead.

Key Responsibilities

• Design and implement the Population Health Strategy to align with state quality strategies and population needs
• Lead the development and monitoring of action plans based on performance metrics and ensure compliance with NCQA and state requirements
• Conduct population health assessments and evaluate program effectiveness through data analysis and reporting

Required Qualifications

• Master's degree in public health or a related field is required
• Minimum of five years of relevant work experience in program evaluation is required
• Experience using the PDSA (Plan, Do, Study, Act) model for improvement is required
• Previous healthcare or managed care experience is preferred
• Six Sigma or project management certifications are preferred
