Job Summary

A company is looking for a Care Transition Nurse (RN) - Population Health.

Key Responsibilities

• Identify, enroll, and manage patients transitioning from acute care to community settings
• Develop and implement transition care plans to enhance healthcare outcomes and ensure safe discharge
• Collaborate with healthcare teams to execute patient-centered care plans and track referrals to resources

Required Qualifications

• Associate's Degree in Nursing; Bachelor's Degree (BSN) preferred
• Active Registered Nurse license in the state of patient care
• 2-3 years of experience in acute care, home health, or case management
• Case Management certification preferred
• Knowledge of patient care across various age groups
