Registered Nurse Case Manager

Job Summary

A company is looking for a Full Time Registered Nurse, Adult Complex Case Manager (Very High Risk/Transition of Care) - Remote.

Key Responsibilities

• Conduct thorough assessments of members' health concerns and prioritize medical needs
• Develop and communicate comprehensive personalized care plans in collaboration with members and their care teams
• Guide members to appropriate facilities and resources based on their medical needs

Required Qualifications

• Bachelor's degree in nursing (BSN) or equivalent
• Current/active Certified Case Manager License (CCM) or commitment to obtain certification within one year of hire
• Minimum of 5 years as an active Registered Nurse with a current unrestricted license
• At least 2 years of direct care or case management experience in relevant specialty fields
• Minimum of 1 year telephonic case management experience
