Care Management Associate

Job Summary

A company is looking for a Care Management Associate (Remote).

Key Responsibilities

• Conduct initial review and triage of Care Team tasks
• Screen patients to identify needed medical services and coordinate referrals
• Support the development and implementation of care plans and coordinate service delivery

Required Qualifications

• 2-4 years of experience as a medical assistant or office assistant
• Computer literacy, including proficiency in Excel and Microsoft Word
• High School Diploma or G.E.D. required
