A company is looking for a Care Management Support Coordinator II to assist with administrative care management activities.
Key Responsibilities
- Conduct outreach to members to support care plan next steps and connect them with health plan resources
- Serve as the primary contact for member and provider inquiries, explaining care plan procedures and protocols
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications

- 1 - 2 years of related experience
