Key Responsibilities
- Support the Care Support Partner in delivering coordinated services to clients and families
- Serve as an initial point of contact, responding to inquiries with empathy and professionalism
- Collect and input vital client information into the secure client portal and assist with documentation management
Required Qualifications
- Bachelor's degree or equivalent experience in Human Services, Healthcare Support, Customer Service, or related fields
- 2 years of experience in a client-facing or service support role, preferably in a healthcare, nonprofit, or social services setting
- Demonstrated commitment to empathy, confidentiality, and professionalism
- Ability to work independently while contributing to a collaborative team
- Foundational knowledge of care coordination services or human services environments
