Key Responsibilities

• Provide monthly care coordination through planning, facilitation, and advocacy for patients' health needs

• Maintain patient chart compliance and document relevant information to support care coordination

Required Qualifications

• Licensed and registered to practice as a Licensed Practical Nurse in a COMPACT state
• LPN degree from an approved program required
• Must pass a background check
