North Carolina Licensed Case Manager, RN

Job Summary

A company is looking for a Remote Case Manager, RN.

Key Responsibilities

• Monitor utilization patterns and facilitate appropriate health care delivery for selected members
• Perform comprehensive assessments of member's health status and support needs
• Collaborate with healthcare teams to develop and coordinate treatment and discharge plans

Qualifications

• RN or LPN with a North Carolina license required
• Prior Telephonic Case Management experience is a plus
• 3+ years of inpatient care experience
• Critical care experience, including med-surg and ICU
• Must reside in the EST time zone
