Job Summary
A company is looking for a PCA Service Coordinator.
Key Responsibilities
- Facilitates timely discharges and care transition support services through collaboration with stakeholders
- Conducts functional needs assessments, develops service plans, and provides service coordination
- Maintains information about community resources and assists Medicaid consumers during the care transition process
Required Qualifications
- Bachelor's degree in a related field or equivalent combination of education and experience
- Qualified Mental Health Associate (QMHA) Certification required
- 2+ years of clinical experience
- 1+ year of care coordination experience preferred
- Knowledge of Oregon's Home and Community Based Services preferred
