Job Summary

A company is looking for a PCA Service Coordinator.

Key Responsibilities
  • Facilitates timely discharges and care transition support services through collaboration with stakeholders
  • Conducts functional needs assessments, develops service plans, and provides service coordination
  • Maintains information about community resources and assists Medicaid consumers during the care transition process
Required Qualifications
  • Bachelor's degree in a related field or equivalent combination of education and experience
  • Qualified Mental Health Associate (QMHA) Certification required
  • 2+ years of clinical experience
  • 1+ year of care coordination experience preferred
  • Knowledge of Oregon's Home and Community Based Services preferred
