Pennsylvania Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to support their Pennsylvania Health & Wellness Medicaid LTSS population.

Key Responsibilities

• Evaluate member needs and facilitate care management plans for optimal healthcare outcomes
• Coordinate with providers and community resources to address members' unmet needs
• Monitor member progress and update care plans as necessary to meet goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse State Licensure and/or Compact State Licensure required
• Bilingual English/Spanish is extremely helpful
• Remote work capability with a schedule of Monday - Friday, 8 AM - 5 PM Eastern Time Zone
