RN Care Navigator

Job Summary

A company is looking for an RN Care Navigator responsible for case management in kidney health management.

Key Responsibilities

• Manage in and outbound calls to provide service to members
• Educate members on kidney health and related conditions, including dialysis and transplant options
• Coordinate care among patients, their support networks, and healthcare providers

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
