Key Responsibilities
- Provides telephonic and virtual outreach and engagement services to eligible members
- Collects and documents member information, including consent and emergency contacts
- Initiates the intake process and communicates urgent needs to care teams
Required Qualifications
- 1 year of experience in a similar role
- Bilingual preferred
- Familiarity with data management and tracking systems
- Ability to work collaboratively with care teams
- Experience in providing customer support or outreach services
