Job Summary

A company is looking for a Disease Specialty Coordinator for the Pediatric Heart Institute.

Key Responsibilities

• Establish and maintain a disease-specific network with community and outlying hospitals
• Ensure compliance with TJC Certification/Accreditation and collaborate with relevant departments
• Facilitate Continuum of Care by conducting community educational activities and tracking patient outcomes post-discharge

Required Qualifications

• Licensure as a Registered Nurse
• Minimum of 3 years of relevant work experience
• Bachelor's degree in a related field
• Experience in maintaining compliance with accreditation standards
• Knowledge of current national practice guidelines
