Virginia Licensed Transition Coordinator

Job Summary

A company is looking for a Regional Transition Coordinator - Remote.

Key Responsibilities

• Participate in discharge planning for members transitioning from long-term care facilities to the community
• Collaborate with community resources and care coordinators to facilitate safe transitions for members
• Provide follow-up support during the first year after discharge and adjust transition plans as needed

Required Qualifications

• 4+ years or a higher degree in a behavioral health related field with 3+ years of direct experience, or LPN with 3+ years of direct experience
• 2+ years of experience serving individuals with behavioral health conditions
• 1+ years of experience with local behavioral health providers and community support organizations
• 1+ years of experience with MS Office applications
