Job Summary

A company is looking for a Clinical Program Coordinator RN to provide care coordination services to members.

Key Responsibilities

• Provide care coordination services including disease management and case management for members with complex health needs
• Assist with triage, referral, transition of care planning, and end-of-life care planning
• Support members in managing their health plan benefits and finding alternative services when necessary

Required Qualifications

• 5 years of clinical nursing experience
• Current unencumbered Registered Nurse License in the state of residency upon hire
• Experience collaborating with physicians in patient care management
