Bilingual LPN Care Coach

Job Summary

A company is looking for a Bilingual LPN Care Coach to work remotely and support patients enrolled in Medicare's Chronic Care Management Program.

Key Responsibilities:

• Utilize care management software to conduct monthly calls with Medicare patients with chronic conditions
• Build rapport with patients and coach them on self-management strategies and health improvement
• Implement and improve the Plan of Care by updating patient information and connecting them with community resources

Required Qualifications, Training, and Education:

• Fluent in both English and Spanish
• Current, unrestricted LPN Compact License or New Mexico LPN license
• 5+ years of experience as a Nurse
• Proficiency with EPIC electronic health record and web-based applications
• Ability to work independently while meeting performance metrics
