California Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical health needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and monitor care plans, collaborating with providers to address unmet needs
• Provide education and resource support to members and their families regarding healthcare options and benefits

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• CA RN License required
• Registered Nurse state licensure and/or compact state licensure required
