California Licensed Case Manager

Job Summary

A company is looking for a Case Manager (Part-time - Night Shift).

Key Responsibilities

• Comply with Case Management Department's policies and ensure adherence to health plan standards
• Coordinate discharge plans and conduct initial and concurrent reviews of medical records
• Assess and evaluate options and services to reduce hospital readmissions

Required Qualifications

• Active Licensed Vocational Nurse (LVN) or Registered Nurse (RN) license in California
• Experience using Microsoft applications such as Word, Excel, and Outlook
• Hospice and Home Health Nursing experience is a plus
