Care Coordinator

Job Summary

A company is looking for a Care Coordinator to facilitate client care and manage referrals within behavioral healthcare settings.

Key Responsibilities

• Serve as the primary point of contact for partnerships requiring concierge care and manage referrals for Intensive Treatment Services (IOP and PHP)
• Track, manage, and document the lifecycle of all referrals to ensure timely outreach, placement, and follow-up
• Coordinate with internal teams and external organizations to enhance care continuity and successful client engagement

Required Qualifications

• High school diploma or equivalent required; Associate's degree in a healthcare-related field preferred
• Minimum 2-3 years of experience in care coordination or referral management within behavioral healthcare or community health settings
• Familiarity with higher levels of care (IOP, PHP, inpatient) and behavioral health systems of care
• Experience working with electronic health records and case tracking systems
• Experience working remotely and/or with virtual care models is a plus
