Key Responsibilities
- Engage with members telephonically to coordinate care, dedicating 50-75% of the day to this task
- Develop individualized care plans in collaboration with members to address their needs
- Document care management activities and collaborate with the Interdisciplinary Care Team to overcome barriers to care
Required Qualifications
- 2+ years of experience in a health-related field
- 2+ years of customer service experience
- Proficient in Microsoft Office Suite
- Access to a private, dedicated workspace for conducting work
- Associate's degree in a health care-related field or relevant experience
