Key Responsibilities

• Assess, plan, implement, and coordinate case management activities for members
• Utilize clinical tools to evaluate members' needs and develop proactive care strategies
• Collaborate with healthcare teams to overcome barriers and enhance member wellness

Required Qualifications

• Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
• 3+ years of clinical/nursing practice experience
• 1+ year of medical case management, care coordination, or discharge planning experience
• High-speed internet access and ability to hard wire computer
• 3+ years of experience with personal computers and MS Office Suite applications
