Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Perform outreach to members to support care plan next steps and connect them to resources
  • Serve as the front-line support for member and provider inquiries and document member records
  • Assist with member onboarding and maintain compliance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED required
  • 1 - 2 years of related experience preferred
  • Knowledge of health plan activities and community resources
  • Ability to document and maintain non-clinical member records
  • Understanding of Social Determinants of Health (SDOH) needs
