Georgia Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN) who will develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to achieve optimal health outcomes
• Collaborate with providers and community resources to address members' unmet needs
• Monitor and update care plans based on member progress and changing conditions

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Certified Diabetes Educator - Preferred
